NHS Health Checks

These are offered to patients aged between 40-74 years. This helps you and your GP reduce your risk of heart disease, kidney disease and type 2 diabetes.

Antenatal

Our midwife runs a clinic at the surgery every Wednesday. The midwife will monitor you during your pregnancy and provide useful information for a health pregnancy. If special care is required she will refer you on for specialist care.

Cervical Smear Screening

Women aged 25-64 years are invited for cervical smears by the local screening service. If you are aged 25-49 years the cervical smear is taken every 3 years; women aged between 25-65 years are invited for screening every 5 years.

Chronic Disease Management

Our specialist nurse will review patients in her chronic disease clinic; conditions such as COPD (Chronic Obstructive Pulmonary Disease), diabetes and hypertension that require long-term management

Contraception & Family Planning

The nurses see patients for contraception advice regarding the contraceptive pill and injections. We also run a clinic every Thursday where we can offer the contraceptive implant or coil to be fitted, this is run by Dr Ijeneme.

Counselling

Talking Mental Health service hold regular clinics on Mondays & Tuesdays, other organisations may request to see our patients at the practice including Treetops, Derbyshire Substance Misuse Service.

Minor Surgery

The practice offers some minor surgeries and also steroid injections in the joints.

Phlebotomy Service

We provide appointments for blood tests every morning daily for testing at both Nottingham and Derby hospitals.

Stop Smoking

We have a stop smoking service run by our nurse, Louise Locastro, which you can make an appointment to see her for help and advice.

Substance Misuse Clinic

This is run by Dr Fogarty in conjunction with the Derbyshire Substance Misuse Service; and is by specialist referral only.

Travel Clinic

This is a private service and is not covered by the NHS contract. Patients are asked to come and complete a travel questionnaire and book an appointment 2 weeks after returning their completed questionnaire. We request patients see the nurse at least 6-8 weeks before you travel. If you are travelling sooner, it is at the discretion of the nursing team and also if we have the appointments available with one of them to see you before you travel as to whether we will be able to provide this service and if not they will advise you to attend a private travel clinic.

Vaccination

This includes child and adult immunisations as per the NHS vaccination schedule recommended by the Department of Health.

Children

Please have a responsible adult accompany any child under 14 years. If anyone other than a parent/legal guardian is bringing a child for their immunisation appointment please make sure they bring written permission from their parent/legal guardian otherwise we will not be able to administer the immunisations.

Certificates

The certificates for DWP purposes are free, however if a duplicate is required this will incur a charge, all private certificates will incur a charge, when requesting these, please state the duration or dates and reason for absence from work. Charges for private letters and notes are available at the reception, these include: insurance claim forms, private sick notes, vaccination certificates, medicals for HGV/ adoption/fostering etc.

Please use the letter below as a sickness certificate as Park View will not be providing a medical certificate for patients requiring self-isolation for coronavirus;

Chaperone

For all intimate examinations we offer a chaperone, if you are aware you will require one please inform reception or if not please ask the clinician you are seeing for one.
