New Patients

When you apply to join our practice list you will be asked to complete the following forms:

These forms are available to print off from the links above. We ask that they are handed into the reception team who will ensure that all the necessary information has been completed.

You will also need to bring Photo Identification with you (Passport, Driving Licence etc).

In summary, to register you should complete GMS1, New Patient Registration form, ‘Consent for Data Sharing’ form & have photo ID.

As part of your registration with The Golden Brook Practice, you will be required to book and attend a new patient check appointment with one of our practice nurses. This must be done when you return your paperwork to the Surgery. Failure to attend for this may result in your application form being declined and your application to join the Practice terminated.

PLEASE NOTE: All new patients must see a member of the nursing team for a New Patient Medical once they have registered. Please book this appointment at reception.

We want to get better at communicating with our patients. We want to make sure you can read and understand the information we send you. If you find it hard to read our letters or if you need someone to support you at appointments, please let us know.

We want to know if you need information in braille, large print or easy read. We want to know if you need a British Sign Language interpreter or advocate.

We want to know if we can support you to lipread or use a hearing aid or communication tool. Please tell the receptionist when you arrive for your next appointment, or call us on 0115 8554200 between 1pm and 5pm.
